Important Notes

Please note that the ELD and accessories were initially provided free of charge. If you decide to cancel, both the device and its accessories must be returned to us. A Refundable ELD Return Deposit of $95 per device will be charged to your billing account. This deposit will be fully refunded once we receive the device in good condition.

Once this form is submitted, a prepaid return label will be sent to your email address, which can be used to ship the device(s) back to us.
